Individuals who request the “Live” aspect of Triumph receive life coaching, which consists of working with an individual to formulate life goals and scheduling home visits to assist with implementation of these goals. The Life Coach also assists with activities such as home maintenance, nutrition education, menu planning, grocery shopping, scheduling medical appointments, budgeting, and any referral needs. The Life Coach records these observations and activities for further review and to document progress.
Individuals who request the “Work” aspect of Triumph receive supported employment services. This process begins with a formal work assessment, followed by job development, job placement, job coaching, and long-term support. Triumph has had great success in job placement by using a “staffing agency” approach to employment. The Employment Specialist from Triumph approaches employers with a presentation of the benefits of working with individuals with disabilities. This model has been crucial in changing the way employers view individuals with disabilities, as well as offering a 85% employment rate for individuals receiving supported employment through TRIumph.
Individuals who request the “Play” aspect of Triumph receive social activity coordination. This service includes an organized weekly activity with other individuals receiving the play service. A Staff Member of Triumph attends these activities to encourage healthy social interactions. Each individual receives a monthly calendar listing activities and events offered through Triumph, as well as local events that may be of interest. In addition, Triumph Staff Members assist individuals in developing their interests and finding other individuals who have similar interests.
